Details
Notification
of Address and Name Changes
Sections 140, 141 and 142 of the Health Practitioners Competence Assurance
Act 2003 requires that psychologists registered with the New Zealand
Psychologists Board must promptly give the Board's Registrar written
notice of any changes to
current postal address, current residential address, and (if applicable)
current work address. (Note: this information will remain
confidential). Use the form below to change your address details.
Psychologists who make any changes to their name after registering with the
Board (e.g. marriage) should notify the Registrar in writing of the new name
within 1 month of the change.
On written application of a registered psychologist, the Board may direct
the Registrar to cancel their entry in the register relating.
NOTE: The Board
must form time to time publish the Register of Psychologists.
The NZ Register of Psychologists may include address information about a
psychologist who
has not objected to the Board in writing to the inclusion of that
information, or who has withdrawn a previous objection.
Cancellation
of Register Details - "Gone. No forwarding address!"
If the Registrar does not receive a reply to a letter posted to a
psychologist on the NZ Register of Psychologists, within 6 months after it was posted, or if the
letter is returned to the Registrar undelivered, the Registrar may give that
psychologist notice in writing at his or her last known address that their
entry in the register be cancelled if the psychologist does not respond within
10 working days after the date of the notice.
If the Registrar does not receive a reply within 10 working days after the
date of the notice, the Board will be notified, and it may direct the Registrar
to cancel the entry relating to that psychologist.
Restoration
of Register Details
If a psychologist's entry in the register has been cancelled, the
psychologist can apply in writing to the Register to have that entry
restored. Exceptions to restoring a psychologist's register
entry may occur under the following circumstances:

whether that person could not be registered in accordance with section 16
of the Act (i.e. fitness for registration);

whether they were subject to pending disciplinary proceedings under
Part 4 of the Act; or
-
whether that person was subject to an order under section 101(1)(a) of
the Act.
Changing Register Details
Change of Name
Send a certified copy of the Deed Poll Certificate, Marriage Certificate or
other official document showing change of name details, with covering
letter to the Board.
Change of Title
Registered psychologists who have since registration graduated with a PhD may change
their title on the Register of Psychologists to Dr. A certified copy of the PhD document
should be sent to the Board along with a covering letter.

your new address details and include your registration number and
