The forms below are available in PDF format to download from this webpage, or you may request a copy by email from the Deputy Registrar (Registration) at any time.  Click on the title of the form listed below to download the PDF version.

(You'll need Adobe Acrobat Reader to read the PDF files; it's available to download for free. It may take a while to download.)

Important: Please read these forms carefully before completing your application.  An incomplete application, statements or information given in any application, which is materially false or misleading, may result in deferment of the right to practice or refusal of registration.

The Board's current fees can be seen here.

Note: All applications received by the Board's office for processing will be dealt with in the order they arrive, subject to all evidential information requested being provided by the practitioner.  Staff treat all applicants fairly and requests for 'fast-tracking' an application should be withheld.  The Board will issue a receipt on payment.

HPCAA Registration: Application to be Registered as a Psychologist in New Zealand

This form is for people applying to be registered within a scope of practice by the New Zealand Psychologists Board.  This includes New Zealand trained applicants, overseas trained applicants, and students in their final post graduate qualification year.

Evidence required with an application for registration includes qualifications, 3 character references, a curriculum vitae, record of criminal convictions, and the signing of a statutory declaration.

The Board will assess whether applicants are fit and competent for the scope of practice (based on evidence provided with the application).  Applicants who are deemed fit and competent are registered and must apply for an Annual Practising Certificate if intending to practise.  [see Practising Certificate Application form below].

If the applicant is found not fit and/or not competent they will not be registered.  (Applicants may appeal Board decisions. See Appeals.)

TTMR Registration Application form

This form is for psychologists registered and able to practise in a state or territory of Australia (except Western Australia) applying for registration by the New Zealand Psychologists Board under the Trans Tasman Mutual Recognition Act (TTMRA) 1997.  The Registrar will undertake required checks with the relevant Australian authority.  Once registration has been granted, the psychologist must apply for an Annual Practising Certificate if he or she intends to practise in New Zealand (see Practising Certificate Application form below).

TTMRA registered psychologists have a psychologist scope of practice.  They may apply to seek a change to this scope using the Vocational Scope of Practice application form.

If the Registrar identifies reasonable grounds under section 27(1) of the Act, the application will be referred to the Board and conditions on the practising certificate may be issued.

Practising Certificate 2010-2011 Application Form (75 Kb)

This form is to be used by psychologists who are already registered for the purpose of applying for a practising certificate. Applications must be sent to the Board's Registrar on this form and with full payment. A statutory declaration is required which must be signed and witnessed.

The Board will not issue a certificate unless it is satisfied that the applicant meets the required standard of competence. Under section 30(3) of the HPCA Act 2003, the practitioner is to be regarded as holding an APC from the date when the Board received the application until the date the APC is issued, or the practitioner is notified that it will be declined.  This means if you haven't received your APC, but you have completed the correct form, sent it into the Board with your payment, and it has been received by the Board and the APC fee deducted from your account, you are deemed to hold that APC.

Vocational Scope of Practice Application Form (74Kb)

For information regarding the vocational scopes  of practice [click here]

This form is to be used by psychologists who are registered for the purpose of applying for a vocational scope(s) of practice - "Counselling Psychologist" and/or "Clinical Psychologist" and/or "Educational Psychologist". The Board urges all psychologists to carefully consider the implications of holding a vocational scope of practice before making application.  Psychologists in New Zealand are bound by their Code of Ethics to practice only in those areas in which they are demonstrably competent, and to maintain competence in their area(s) of practice.

A statutory declaration is required for this application.  Statutory Declarations made in New Zealand must be made before a person entitled under the Oaths and Declarations Act 1957 to take statutory declarations, e.g. Justice of the Peace, solicitor, notary public, Registrar or Deputy Registrar of the High Court or any District Court, authorised officer in the service of the Crown, or any member of Parliament.

Non Binding Assessment of Qualifications

This form is used primarily by overseas-trained people considering registration as a psychologist in New Zealand.  The initial non-binding assessment of qualifications will give applicants an idea of how the Board rates the equivalence of their qualifications in relation to the minimum standard of qualifications required for registration in New Zealand.  The results of a non-binding assessment do not guarantee full registration.

Certificate of Registration Payment Form

This form is to be used by registered psychologists wishing to purchase a Certificate of Registration, which shows the practitioner's name, scope of practice in which they are registered and date of registration.  Certificates are generally used for open display to increase public awareness of a psychologist's status of registration and to enhance public confidence in the professional practice of psychology.
